Evaluation forms
One way to assist in the development of the Forum is by receiving feedback from you as members on how the Forum is developing, on the meetings and any other information that you think can help the Forum to develop and grow.  A short questionnaire for this purpose is attached and is handed out at Forum meetings. These forms in the future will be sent out electronically to all members on a regular basis.
