Meals and wheels expression of interest form

This form can be completed to express an interest in using Spelthorne's Meals on Wheels service. 

Referrals to the service can be made by Social Services, General Practitioners or other health care professionals such as district nurses, hospital discharge coordinators or by friends and family. The service is specifically aimed to help people who are unable to feed themselves, so customers are required to meet a set criteria which will be assessed on receipt of this form.

Criteria for referral, the client must be:

  • living alone or are alone in the daytime during mealtimes.
  • unable to prepare or cook for themselves.
  • housebound and unable to shop for a main meal.
  • at risk of malnutrition; not motivated to cook.
  • unaware of time and/or need for regular meals, where there is clear evidence of confusion.

If referral to the service is from social services or a GP, meals can commence the next working day. This will be followed by a home assessment within 20 working days.

If referral to the service is by a family member/friend  a home assessment is required before meals can commence. We have 10 working days to complete this assessment.

Assessments will be arranged by appointment via our Day Centre Managers. All clients will continue to be monitored by the delivery drivers and will be reassessed annually, or sooner if their requirements change.
