	The Spelthorne Health and Social Care Partnership is a multi-agency partnership, which aims to improve health and social care in Spelthorne with appropriate services for everyone. It promotes consultation with health service users and carers through a number of forums that feed into the Partnership: Spelthorne forum for older people Spelthorne learning disabilities forum Spelthorne physical and sensory disabilities forum Children with disabilities forum Patient forum Mental Health forum Spelthorne Carers Forum The Partnership deals with the healthy Spelthorne section of the Spelthorne Community Plan, planning how to meet the targets and monitoring progress.
