Certificate of the Registrar of Death (the green form) or Coroner's Order for the disposal of the body to be buried (white form) must be sent along with the Notice of Interment form.

For the internment of cremated remains a Certificate of Cremation must be produced from the Crematorium.

The burial will not take place without these documents.

Proof of Residency

Proof of residency for the purchaser and/or the deceased must be submitted to the Funeral Director to send to the Cemeteries Office. If booking directly with Spelthorne Borough Council, the Cemeteries Office will advise directly if proof of residency is required.

Please note proof of residency may be required as part of the booking process. Residency qualification (for purchaser and/or deceased) will be determined through one of the following:    

  • Council Tax Demand (current year) indicating that the person occupied a property within the borough
  • Electoral Roll document (current)
  • Utility Bill received at the property (not more than 3 months old)
