Social prescribing supports individuals to take better control over their health and improve their general wellbeing.
It aims to link residents, who have social, emotional or practical needs which cannot be addressed by a GP, to a wealth of different services within the borough.

Spelthorne Borough Council deliver Social Prescribing for residents of Spelthorne and on behalf of Woking Borough Council for residents of Woking.
What we offer
We offer a one-to-one appointment with a Social Prescribing Link Worker who will talk to you about your concerns and factors affecting your health. Our Link Worker will support you for up to 6 weeks and help to identify and organise suitable assistance via a network of services and local community and voluntary organisations.
Our Social Prescribing Link Workers can help you to access support in the following areas:
- remaining independent at home - pendent alarms, meals on wheels, home adaptions
- opportunities for social interaction – social clubs and activities and community centres
- practical help, such as cooking, transport, shopping - carers, transport, shopping, gardening, cleaning
- physical activity - seated exercise and walking groups
- benefits and budgeting advice – debt, universal credit, attendance allowance
- housing options advice - linking people to the housing team and housing register
- low level mental or emotional support – referrals for counselling and support groups
- support around a disability – disability support groups & blue badge applications
- family support – referrals to the foodbank, Homestart
Who can use this service?
The service is currently available to adults over 18 years old, who are registered with a Spelthorne or Woking GP surgery and experiencing non-medical issues requiring additional support.

Once the referral form has been submitted a Link Worker will get in touch to arrange an appointment with you. This can be over the phone, at your home or in a community location.
