Privacy Notice Food Bank Referral Form

Manna, Stanwell Foodbank, Salvation Army, St Saviours, Bags of Food, Surplus for Supper are foodbanks or surplus food distributors within Spelthorne Borough and are all part of the Spelthorne Food and Welfare Network. 

This referral form is provided for organisations to refer individuals/families for food assistance, and the information provided will be shared with one or more of the foodbanks listed above depending on the address of the potential foodbank customer. ­

Those foodbanks are the data Controllers for information provided to them from the form and for any further information that they collect.  

Spelthorne Borough Council is the data Controller for the information collected on the form because the council is providing the technology to make this form possible and act as a central point for the organisations listed at the top of this notice.  The data is stored completely securely and separately from any other personal data that the council holds.

The following information is collected for potential foodbank customers:

Full name

Date of birth

Postcode

Address

Phone number

Referral date

Dietary needs/allergies if applicable

Nappy size if applicable

Baby food if applicable

Information about any pets if applicable

The reason assistance is required and whether that referral is urgent

Whether a referral has been made to a foodbank in Spelthorne before

Your information will be shared with one or more of the foodbanks listed above.

Referrer email address

Referrer phone number

Position held

Your information will be shared with one or more of the foodbanks listed above.

Additional information:

  • The Council will keep personal data provided on the form for a maximum of 6 years after your last contact with the Spelthorne Food and Welfare Network.
  • The foodbanks will have their own retention periods.
  • The Council's lawful basis for using this information is public task, as we have public interest in reducing food poverty.
  • The foodbanks will have their own lawful bases.
